Your treatment plan will include a breakdown of all applicable fees, and we will inform you of all costs before treatment is administered. If you do not have insurance, full payment is due at time of service. We offer a variety of payment options to meet your needs. Our office accepts payments by cash, checks, money orders and Visa, MasterCard and Discover credit cards. Ask our office about financing your treatment through CareCredit. We charge what is usual and customary for our area. You are responsible for payment regardless of any insurance company’s arbitrary determination of usual and customary rates.

All accounts more than 60 days past due are subject to a 1.5% per month service charge. The minimum monthly service charge is $5.00. There is an $8.50 charge for all returned checks.

If your account is turned over to collections, you are responsible for any collection and/or legal fees


Our office is committed to helping you maximize your insurance benefits. Because insurance policies vary, we can only estimate your coverage in good faith but cannot guarantee coverage due to the complexities of insurance contracts. Your estimated patient portion must be paid at the time of service.

As a service to our patients, we will bill insurance companies for services and allow them 45 days to render payment. After that time, you are responsible for the entire balance, paid-in-full. If you have any questions, our courteous staff is always available to answer them.

Some Important things to Remember about Medical and Dental Insurance

  • We are a provider for most insurance plans.
  • We may accept assignment of your insurance plan.
  • If we accept assignment of your insurance plan, payment in full may not be required. However your deductible and co-pay is required at the time of service.
  • Your insurance is a contract between you and your insurance company. We are not a party to that contract. If your insurance company has not paid your account in full within 45 days, the balance will be transferred to your account.
  • Your insurance company may not cover some services. This is not a statement by the insurance company that the service was not necessary but rather a reason for the insurance company to reject payment. You are responsible for payment of all non-covered services.
  • We have contracts with some insurance companies to provide services for their clients at a discounted rate. In this case we will write off the difference between our usual and customary rates and the discounted rates, but you are still responsible for your deductibles, co-pays or co-insurance.
  • Please Note: Dental related procedures and dental implants are not covered by Medicare.
  • Some insurance companies require a referral from a doctor or dentist to a specialist. Without a referral the insurance company will not pay. If your insurance requires a referral and you do not have one at the time of visit, your visit will not be a covered expense and will be your responsibility. We are not allowed to obtain a retrospective referral.
  • For dental implants, we do not accept insurance assignment. Full payment is due at the time of service unless other arrangements have been made. As a courtesy to our patients, we will bill your insurance and reimburse to you any payments made toward your implants by the insurance company.
